Athletic Clearance

Information for Participation in Athletics and Co-Curricular Activities

Athleticclearance.com instructions:

Go to athleticclearance.com to begin creating your account.  You will need to create an account in order to begin.  Once you have done that, please click on the button that says Start Clearance Here! You will then select the year, school, and sport for your athlete.  Please be sure you select the correct school year (2017-2018). You will need to do a separate clearance for each student athlete in your house.  Only one parent account is needed regardless of how many athletes you have at NPHS.  There is a tutorial video on the home page if you need further assistance.

All forms require signatures which can be done as electronic signature of both athlete and parent.  Use the doctor’s physical form on the website.  It must be filled out in its entirety, signed and dated by the doctor, and then uploaded back into the site.  Please scan both pages together or take a picture of both pages side by side, so the two pages are only 1 file to upload.  We also require a copy of the front of the students’ medical insurance card to be uploaded for clearance.  If you upload forms to the “documents library” section of athletic clearance, you will still have to select the file from that location to upload it to your child’s profile.  Saving files to the documents library does not make it visible to the athletic department.  Once all forms have been completed online & personally checked by the athletic department, you will receive a confirmation that your student athlete is clear for their season.

The physical exam is valid for 12 months from date of exam.

  • Students are not eligible to practice, or participate in any sport or activity until all digital forms are completed and the physical and proof of medical insurance is uploaded to AthleticClearance.com.
  • Athletic Office will not fax or email forms to/from parents or doctors.

Academic Eligibility

Students must maintain a 2.0 GPA and be passing 20 semester credits to be eligible to participate in Athletics or a Co-Curricular Activity.  Initial eligibility is based on the grading period immediately before the season of sport. Fall eligibility is based upon the end-of-the-year report card (June, 2017). Eligibility will be reviewed at each grading period.

PLEASE NOTE:  Any student who has transferred to NPHS in the 12 months prior to a season must contact the Athletic Director.

Transportation

Athletes are expected to ride the team bus to and from all athletic events. Transportation fees are due the first week of practice.  Transportation fees for athletics and activities can be paid online through the Student Online Store here: Student Store or by check at the Student Store on campus.  The transportation fee decreases when the same athlete plays more than one sport or activity within the school year.  Siblings cannot be combined for the discount.

1st sport/activity of the school year — $124.00

2nd sport/activity of the school year — $ 71.00

3rd sport/activity of the school year — $ 55.00
